PHR Family Plan - $29.90 per month

Build your family’s individual Personal Health Records and access them at any time, from anywhere! The One Year Membership includes Personal Health Records for (2) two adults and up to (3) children. Your children’s Personal Health Records are free (if under 18 years-old). The PHR Family Plan also includes our Medical Record Assistance Program for medical records from one (1) of each adult’s physicians.

  • All children’s medical records billed at $25 per physician;
  • Additional adult medical records billed at $25 per physician;
  • Our platform also allows you to easily upload as many of your medical records as you’d like into our system.

PHR & SMO Individual Plan - $44.95 per month

Build your own One Year Personal Health Record Membership and combine your membership with (1) one Online Second Medical Opinion! This plan includes your medical records from any (1) one of your physicians through our Medical Records Assistance Program. Additional Patient Records are billed at $25 per physician. Our platform also allows you to easily upload as many of your medical records as you’d like into our system.

  • This plan also includes (1) one Online Second Medical Opinion per year from a committee of Activ Doctors Online’s network of physician specialists within 48 hours of your request.

*Members records must be sufficient and complete enough to provide an opinion within 48 hours.
