The member, through a HIPAA release form, authorizes Activ Doctors Online to confidentially gather records from the health provider. ADO then contacts the physician, collects the medical records and uploads them to the members profile. This includes medical history, prescriptions, vaccinations, allergies, surgery reports, tests and screenings, exams and more. Additional records are billed at $25 per physician, although the platform allows members to easily upload unlimited medical records.
